Patient Registration
If you live within the practice boundary area (most postcodes beginning with NR32, please see the Practice Area page for a full list) you are able to become a registered patient. Please present at Reception with two forms of identification (one of which should be photo ID i.e. passport or driving licence and proof of address could be a utility bill). You will be given a Patient Health Questionnaire and an additional registration form to complete. You can request a Practice Leaflet which gives information about the practice's services.
Children can only be registered if a parent/guardian living at the same address is also registered with the practice.
At the time of registration you will be informed of your named accountable GP. If you are already registered and do not know who your named accountable GP is please ask at reception.
If you are a patient and wish to register for online services which will allow you to access the following- booking/cancelling appointments, ordering repeat medication and viewing your medical record, please ask at reception for a unique identification number and then use the link below to register.
